PROOFS OF RESIDENCY REQUIRED:
- Current Tax Bill or Lease
- Current Utility Bill
- PROOF OF CHILD'S AGE (Birth Certificate or Passport)
- PHYSICAL EXAM & IMMUNIZATION RECORDS (DATED WITHIN THE LAST 12 MONTHS)
Available Mondays, Wednesdays and Fridays beginning March 25, 2020, by appointment only between the hours of 9 - 11 am and 1 - 3 pm.
