    If you are calling your child in sick please notify the school at the earliest possible time at 631-591-4622 or 631-591-4623.  Please have your child bring in a note to the nurse on their return to school.

    Students planning to play a school sport:

    All athletes must have an updated physicalIf your child went to their personal physician this past year please mail a copy of the physical to me as soon as possible. It can also be faxed to 591-4652. All parents and athletes must have all sections of Final Forms completed and signed. Both an updated physical and Final Forms completion is needed to tryout for a sport. The same rules apply for each sports season.

     

    All 9th and 11th grade parents: 

    NYS requires all 9th and 11th grade students have an updated physical. Physicals conducted after September of last year fulfill this requirement. It is also recommended that all 10th graders receive a dental exam.  Physical and dental forms were sent home at the end of last year. If additional forms are needed they can be found on the district athletic homepage. 

     

    New Students: 

    NYS requires all new students and transfer students have an updated physical. Please bring in or send me a copy of their physical if performed within the last year. Physicals conducted after September of last year fulfill this requirement. 

     

    All 12th Grade parents:

    NYS requires all 12th grade students have proof of having been given the Meningoccal vaccine. The vaccine to prevent meningitis.  
    Students entering Grade 12 must have either: 2 doses of Meningococcal vaccine with the booster dose given on or after age 16
    1 dose if your child's first dose was given on or after age 16.

     

    Visual and Hearing Screening Policy: 

    All 11th graders and new students have vision and hearing screenings at the beginning of the school year.  Some students are sent to me during the year as well.  Parents will be notified of impaired results the same day with a referral sent home. Please provide the nurse with those results or recommendations from your follow up with your ophthalmologist.
